CMRG's Wellness programs are customized to the needs of the client population and goals of the Plan. Wellness programs can include a poster-a-month campaign in a break room, monthly newsletters, educational seminars, health screenings or a full-blown health fair.

The overall goal of a wellness program is to educate the group as a whole to the need for life-style changes to achieve a state of wellness. Core Health Services Medical Department works to develop a program meeting the client's needs.

LifeStyles Management is a program which identifies and assists individuals with known and unknown chronic illnesses. Members receive professional health care support in order to self-manage and prevent catastrophic complications. Core’s LifeStyles Management Program combines the best of On-site Health Risk Assessments, Individual Treatment Plans, Personalized Nurse Counseling and On-going Monitoring and Support.

LifeStyles Management Program consists of:

    Step 2 – Individual Treatment Plans
    Step 3 – Personalized Nurse Counseling
    Step 4 – Ongoing Interaction and Monitoring


    Step 1 – On-Site Health Risk Assessments

A comprehensive medical profile is established using the following components: Health Risk Assessment, Health Questionnaire, Physician Records, and Drug Utilization History. Core Health Service Team will come on-site to do a physical evaluation (height, weight, blood pressure), draw blood (cholesterol, blood sugar) and have you complete a questionnaire about your own health and family’s health history. The evaluation takes approximately 15 to 30 minutes and is conducted during the hours of 7 A.M. to Noon. The blood work requires that you do not eat or drink anything after midnight prior to the morning of your health risk assessment. If you are on high blood pressure medicine, please take it at your normal time with a small amount of water.

Step 2 – Individual Treatment Plans

The Core Health Services Team will then evaluate each individual’s Health Risk Assessment and develop, with the member and their physician, an individualized treatment plan. These treatment plans will require you to monitor and report any identified risks. Typically risks are reduced or eliminated through treatment and lifestyle changes. The conditions that Core currently manages are Diabetes, Asthma, Hypertension, Elevated Cholesterol Levels, Obesity and Tobacco use.

Once a treatment plan has been developed, a LifeStyles Manager will mail the at risk member a letter that summarizes their identified health risk(s) and their individualized treatment plan.

Step 3 – Personalized Nurse Counseling

Nurses will be available to assist members in reaching the goals set in the individual treatment plans. We encourage participants to look towards their LifeStyles Manager for personal support and coaching. The nurses will be available for one-on-one counseling concerning nutrition, exercise, cholesterol management, blood pressure control, blood sugar control and a wide variety of other health issues.

Step 4 – On-going Interaction and Monitoring

Implementation of the disease management process is not the final step. Once treatment plans have been discussed and educational materials provided, the LifeStyles Manager will interact and support these members. Support includes phone calls, faxes, written correspondence and use of our online tools.
